Landmark’s physician-led medical group gives patients access to a team of specialists who provide highly coordinated and timely care. This team approach helps:
- treat routine and urgent medical needs in the home,
- identify and address social determinants of health,
- assist patients and caregivers with advanced planning documentation,
- provide education on proactive condition management, and much more.
The cumulative effect of this high-touch care is reduced unnecessary hospitalizations and fewer visits to the emergency room, along with lower mortality rates and costs.
