Landmark Health and Blue Cross & Blue Shield of Rhode Island Collaborate to Bring High-touch, In-home Medical Care to Complex Patients

HUNTINGTON BEACH, Calif.--(BUSINESS WIRE)--Landmark Health and its affiliated medical group (Landmark) and Blue Cross & Blue Shield of Rhode Island (BCBSRI) have come together to enhance access to care and coordination for BCBSRI Medicare Advantage members managing multiple chronic conditions.

Landmark’s physician-led medical group gives patients access to a team of specialists who provide highly coordinated and timely care. This team approach helps:

  • treat routine and urgent medical needs in the home,
  • identify and address social determinants of health,
  • assist patients and caregivers with advanced planning documentation,
  • provide education on proactive condition management, and much more.

The cumulative effect of this high-touch care is reduced unnecessary hospitalizations and fewer visits to the emergency room, along with lower mortality rates and costs.
